SUBJECT:   Bidding of janitorial services for the HGC Head Office

DATE     :    January 29, 2015

As discussed and agreed upon during the Pre-bid Conference of January 27, 2015, the following are hereby amended:

Instruction to Bidders:

  1. Item I.4.A.4 Valid and current Mayor’s Permit issued by the city or municipality where the principal place of business of the bidders is located.

    Bidders with pending application for renewal of Mayor’s permit must submit their application for renewal, receipt of payment and previously issued permit

  2. Item I.4.A.5 shall read as follows: Tax Clearance for CY 2013 per Executive Order 398, series of 2005, as finally reviewed and approved by the BIR.

  3. Item III.4 shall read as follows: Within three (3) calendar days from receipt of the bidder of the notice from the BAC that the bidder has been declared as the bidder with the LCRB, the bidder shall submit the following documentary requirements:\
    1. Certificate of PhilGEPS Registration;
    2.  Income Tax Return for 2013 and Business Tax Returns (Value Added Tax only) for the months of January 2014 to December 2014; and
    3. Certification of Remittance for the period January 2014 to September 2014 and Proof of Remittance for the period October 2014 to December 2014 of the following:
      • Social Security System (to include ECC Contributions)
      • Home Development Mutual Fund or PAG-IBIG
      • Philippine Health Insurance Commission

All other provisions of the Instruction to Bidders not inconsistent herewith shall remain valid and binding.
